Our laboratory contracts with a variety of national and regional insurance carriers to help ensure access to our services; however, each health plan has its own policies regarding benefits, prior authorization requirements, deductibles, co-payments, and coverage limitations.

 

Some insurance plans may be administered by third-party entities, and we may not be considered in-network for all plans or services. Please note that even if we are contracted with your insurance provider, this does not guarantee that all services will be covered in full. Patients may still be responsible for certain costs, including co-insurance, deductibles, or services not covered under their individual health plan.

 

While we make every effort to submit claims and assist with the appeals process where appropriate, the final determination of coverage and reimbursement is made by the insurance company. The list of contracted payors is subject to change without notice. For the most current information on your coverage, we recommend contacting your insurance provider directly or reaching out to our billing team for assistance.
